Accepted Insurance

We are in-network with the following health insurances and submit billing on behalf of our clients:

• Healthy Montana Kids

• Missoula County Medical Benefits Plan

• Montana Medicaid

• Medicare

• Railroad Medicare

• TriCare/TriWest (pending in-network status)

We are out-of-network with the following insurance/s, and will also submit billing on behalf of our clients:

• Humana (including Medicare Advantage plans)

We do NOT accept the following as primary insurance:

• United

• ChampVA

*If you do not see your insurance above, we are happy to invoice you directly for services rendered and provide you with a Superbill to submit to your insurance company for reimbursement.

**Please be sure to review your benefits information with your insurance carrier directly for specifics related to your rehab therapy deductible, co-payments, co-insurance, etc. 

Self-Pay

If we are not contracted with your medical insurance or if you do not have medical insurance, we offer the following self-pay rates:

​

OT/PT/SLP

• $200 per evaluation

• $135 – 45 minutes

• $180 – 60 minutes​​

Clients who elect self-pay agree that no insurance claims will be submitted for these services.

Superbills are available upon request for clients who wish to submit claims independently to their insurance carrier.

Other Paid Services

*$250/hour for school readiness exams including report to family and/or school

*$250/hour plus $.58 per mile from our clinic for attending IEP meetings
